Application for certificates<\/strong><\/span>
\nPlease print out the application form of requesting certificates and follow the application procedure as below.
\n*We do not accept applications neither by phone, fax nor e-mail.<\/span><\/p>\n

-Transcript
\n-Certificate of Graduation\/Completion
\n-Credit Transcripts
\n-Certificate for academic abilities related to teaching licenses
\n-License related certificates
\n-Certificate of degree conferral (For Doctoral degree only)<\/p>\n

Apply at the window<\/strong><\/span><\/h6>\n

(Open from 8:30am to 5pm on weekdays)
\nPlease submit the following documents to Student Support (Window 3) in Academic Affairs Office.
\n-An application form filled with required information
\n-A photocopy of a personal identification \u00a0 e.g. Driver’s license, Passport, Individual Number Card (The number is not required)<\/p>\n

Certificates cannot be issued on the day of application, so please come back to receive them later with your ID.
\nIf you wish the certificates to be mailed, please apply with a self-addressed and stamped return envelope.<\/p>\n

Apply by mail (Domestic only)<\/strong><\/span><\/h6>\n

Please mail the following documents to Student Support \u201cCertificates\u201d.
\n-An application form filled with required information
\n-A photocopy of a personal identification \u00a0 e.g. Driver’s license, Passport, Individual Number Card (The number is not required)
\n-A self-addressed and stamped return envelope
\n*We CANNOT pay postage charge.<\/span>
\n*Return mail will be sent by Japan Post. Any other carrirers cannot be chosen.<\/span><\/p>\n

Application by mail (International)<\/strong><\/span><\/h6>\n

If you are living outside of Japan and in need of certificates, please ask family\/friends or someone you know who is living in Japan to apply on your behalf.
\nIn case you need to apply from abroad, please purchase and enclose International Reply Coupon or the Japanese stamps on the Japan post website.<\/p>\n
